The British Connection

The UK plastic surgery industry has been described as “booming” by the BBC. (1) London is a thriving cosmopolitan city that attracts some of the world’s most experienced and most talented plastic surgeons. And, like so much of its past, the long history of the UK surgical profession is incredibly well-documented. The UK’s first medical school was established in Scotland’s capital, Edinburgh, in 1726. And the governing body of plastic surgeons, the British Association of Aesthetic Plastic Surgeons (BAAPS), was founded over 200 years ago in 1800!

How Popular is Plastic Surgery in the UK?

British people spend an estimated £3.6 billion ($4.3 billion) on cosmetic surgery each year. (2) What’s more, British patients, like Americans, demand the highest standards of their surgeons, and will go to great lengths to find the right person for the job. They also share many of the same “favorite” procedures with us. In fact, the top five surgeries year-on-year are very similar to the United States. The three most in-demand surgeries in the United Kingdom from 2009-2018 were breast augmentations, abdominoplasties, and rhinoplasties(2) which are all consistently popular here in the States too.

Introducing Mr Cohen: A Very British Title

Although we share a common language, the United Kingdom and the United States are different in many ways. And US-based surgeons who practice there may notice a change or two in their everyday lives, particularly when it comes to how they are titled. In London, Dr. Cohen will be referred to by all as “Mr Cohen.” And in fact, this is the case for every male surgeon in the United Kingdom and the Republic of Ireland! To Americans, this may seem like some kind of terrible mistake, a clerical error, or even a slight. But it isn’t! In fact, the title itself has a very interesting history. 

Physicians vs. Surgeons in Georgian Britain

Back in the 18th century, like Americans today, British physicians used the title “MD” to indicate their status as medical doctors. However at the time, these credentials were easily bought overseas with minimal academic effort for around £20 (about $24). In today’s money, that’s still only about £800 ($975)! Comparatively, surgeons did not usually possess a formal qualification for the work they did. In fact, quite the contrary. They were not considered professionals, nor were considered peers to physicians, and were seen more than anything else as “the mere operators to be put into operation by their directors [physicians]”.

Then over time, things started to change. At the turn of the 19th century, the Royal College of Surgeons was founded. This meant that surgeons could finally acquire an academic qualification called an MRCS (Membership of the Royal College of Surgeons) as proof of their mastery. Since then, the unique title bestowed only to surgeons, has recognized and honored a longstanding tradition in the British Isles. The title of “Mr” acts as a badge of honor that shows they are qualified doctors in their own right, and distinguished as separate from physicians. (3)
